Review of recent loco failure
cases revealed glaring lapses in existing Shed maintenance practices.
1.
Bad workmanship(BWM)
cases
are on rise. No responsibility is fixed on defaulting staff/ supervisors.
Action taken & details of punishment imposed against all BWM cases should be reviewed.
2.
Proper failure
investigation-
failed locos on arrival in Shed are not thoroughly checked by concerned
sectional supervisors & Officers. Action plan is prepared on assumptions
& is not based on root cause of failures after detailed investigation.
Casually prepared action plan leads to repeated failures for same fault.
3.
Lack of checking of
critical parameters & critical work by sectional supervisors is causing line
failures. Super checking of critical schedule items by officers and supervisors
with endorsement on schedule form should be ensured. Critical scrutiny of
schedule forms by officers daily must be done.
4.
Non-supervision of
non-schedule work/re-work.
Tendency of loco failures after re-work in Shed is major cause of concern.
5.
Tendency to release
locos at night without checking by Supervisor & Officers.
6.
Skill
& knowledge of staff in failure area not addressed. Things are allowed to
drift & failures recur due to same fault again and again. Staff who are not
competent are booked to do critical job leading to line failures.
7.
Lack
of super-checking by Officers and Supervisors of all ready locos.
Shed in charges must tackle
the above issues immediately and ensure that such lapses don’t recur again.
